Defect Reporting

Defect reporting provides a tool to follow the process of opening a defect, logging the required course of action, the resolution of the defect and closing of the defect report. Defects can be assigned a priority level from ‘ship cannot trade’ down to non-urgent break downs.

Some vessels are subject to Defect Reporting, this can be imposed by Flag, Class or statutory requirements such as ISM or Mini ISM. Some fleet operators choose to use defect reporting as a tool for flagging up and logging defects or non-conformity process and resolution.
When a defect is opened it is automatically emailed to the vessels shore contact.

Open a defect report for a part

A defect report can be open via the part dashboard or by going to the defect reporting page.

Via a part dashboard

Select all parts from the parts tab and open the part you want to create a defect report for. You will see the defect reports box, just click the ‘add report’ button and follow the instructions on screen. You are taken to a form, the part will already be selected. Complete the fields and then click the blue “Open” button at the bottom of the page. The report is now open, it will be listed on the main defect reporting menu and also be emailed ashore.

Via the defect reporting page

Go to the defect reporting page and in the top right of the screen click the blue ‘open new’ button. You are taken to a form, first select the part affected. Complete the fields and then click the blue “Open” button at the bottom of the page. The report is now open, it will be listed on the main defect reporting menu and also be emailed ashore.

Opening defect reports for items other than parts

On the open defect report form, go to the bottom of the list of parts, you will see a section called “Other” You can choose which heading to include the report under such as “Deck” you can then detail the part or area in question, this is saved in the system for future repeated use. Complete the fields and then click the blue “Open” button at the bottom of the page. The report is now open, it will be listed on the main defect reporting menu and also be emailed ashore.

View an open defect report

Open defect reports can be seen under each parts dashboard and by going to the main defect reporting section.

Update an open defect report

Click on the ‘i’ icon next to a defect report this will open the report where it can be updated. Add notes, attach documentation or photos and click the blue ‘update report’ button.

Closing a defect report

To close a report first view it, at the top right of the page click the ‘close report’ button. You are asked to confirm appropriate corrective action has been taken.

Edit a defect report

To edit the initial defect report, first view the report. At the top right of the page click pen icon, this will bring up the original options which can then be amended and saved.

Delete a defect report

To delete a defect report, first view the report. At the top right of the page click trash icon, this will give you the option to delete a report. Not all users can delete reports, such as those operating in a fleet environment only management can delete open reports.

Filter defect reports

Go to the main defect reporting page.
The list of defect reports is fully searchable based on the following criteria:
Status (Open/Closed)
Priority
Part/Category
For example you may wish to view all Closed High Priority defects relating to the Deck department. Click “List Filters” and select what you want to view and then click “Apply” you will be presented with all defects fling within your chosen filters.
To view all defects again, go back into the “List Filters” menu and choose “Clear Filters”.

Export a PDF for print or email

From the main defect reporting section, once you have selected your chosen filters to view the appropriate defect reports you can click the ‘PDF Export’ button.
